Athlete Registration

1). Acalanes Sports Registration (must be done before you start playing):

3). Volunteer Driving Form (must be filled out and filed with AHS office prior to driving athletes to/from games):

In order to participate in any Acalanes sport, a physical clearance is required, only one clearance is required for all sports during the school year.

You must complete the physical clearance prior to August 9th, the first day of practice otherwise you will not be able to participate
Attached are the instructions and the clearance form(clearance form) the physician needs to complete
